Family Navigator-PSB Clinical Coordinator FTE 40 hours, Mon-Fri, no weekends

JOB SUMMARY: The Family Navigator (FN) is a Social Worker with comprehensive knowledge of trauma treatment modalities, crisis intervention, and case management. The FN demonstrates critical thinking and utilizes social work ethics to assess and meet the needs of patients and their families/caregivers by providing care coordination throughout the care continuum.

FN assists in improving the clinic care experience and preparedness for treatment by orienting and educating patients, families, and caregivers to the healthcare system and multidisciplinary team member roles and resources. FN will assess and address patients’ barriers to care, partner with patients/families to address their concerns, issues and needs, and facilitate appropriate referrals. FN will serve as the central point of contact to the patient, facilitating collaboration among all team members and ensuring an individualized approach throughout the length of the patient’s needs. FN will demonstrate commitment to quality patient care, implement creative and innovative ways to meet diverse patient needs, and demonstrate ability to prioritize utilizing organizational skills and clinical judgement. FN will promote a consistent, holistic plan of care to enhance the quality of life of patients, families and caregivers through assessment, teaching, communication, support, and the practice of family-centered care. FN will work to influence both positive patient outcomes and system outcomes through improved interdisciplinary communication, patient retention, and community partner collaboration.

The Family Navigator in the PSB Clinical Coordinator position will oversee a multi-level response to PSB for children in Multnomah County. The PSB Clinical Coordinator will lead the Multnomah County PSB committee, manage CARES Northwest’s clinical response to PSB referrals, and strengthen Multnomah County’s systemic response to PSB through outreach, engagement and collaboration in the community.

QUALIFICATIONS:

Education: Master’s degree in mental health field, including Social Work, Psychology, Counseling, or Marriage and Family Counseling.

Experience: Minimum one-year post-MSW experience in one or more of the following areas required: Healthcare, family service, child maltreatment, case management, or coordination of home-based services. Bilingual in English and Spanish is preferred.

Licensure/Certification: Licensed Clinical Social Worker (LCSW) or Clinical Social Work Associate (CSWA, working toward LCSW), Licensed Master’s of Counseling (LPC) or MA/MFT working toward LPC (LPC-Intern) or LMFT required within six months of hire date for employees who perform this job in the state of Oregon.

Child Abuse Interviewer FTE .60 (24 hours per week)

Job Summary: The Child Abuse Interviewer is responsible for developing a medical diagnosis of [possible] child abuse by participating with a medical examiner in an evaluation process that includes a physical examination and videotaped interview.  Interviewer also assists in developing an appropriate follow-up treatment plan for the child and family as well as providing training and presentations on child abuse.

Qualifications: EDUCATION/EXPERIENCE:

MSW, MA, or MEd in social work, counseling, or related field.  Minimum two years professional experience relating to child abuse issues, with preference to those with experience interviewing children about abuse.

SPECIAL SKILLS & ABILITIES:

Excellent verbal and written communication skills.  Ability to relate well to children and adults in crisis.  Strengths in working as a part of a multidisciplinary team.  Some positions within this classification require fluency in Spanish.

Requires Licensed Clinical Social Worker (LCSW), or Clinical Social Work Associate (CSWA) actively working toward LCSW licensure, or Licensed Professional Counselor (LPC), or Registered LPC Intern actively working toward LPC licensure. 

Office Assistant-Culturally Responsive FTE 40 hours a week, M-F (no weekends)

Responsibilities: Provides complex and diversified clerical and office support that requires advanced administrative skills and knowledge. Duties will vary depending on working environment (clinical vs. non-clinical). Responsibilities may include typing, transcription, file management, scheduling procedures and/or meetings, physician billing, ordering supplies and operation of office equipment. Work is generally performed independently requiring judgment and problem-solving skills under limited supervision. May assist with orientation of new personnel. May act as a resource person for employees regarding staffing and payroll issues.

English and Spanish fluency are essential functions for this position.

Qualifications

EDUCATION:

High school diploma or equivalent.

EXPERIENCE: Three years in an administrative support role requiring judgment and performance of a wide range of secretarial and/or administrative functions.  Knowledge of Microsoft Office Products including Word, Excel and Outlook required.  Hospital or healthcare experience preferred.

SKILLS:

Keyboard skills and ability to navigate electronic systems applicable to job functions. Budget skills and the ability to organize and work independently. May require ability to transcribe reports, forms, and correspondence, including the use of technical and medical terminology, from longhand or dictation. Ability to compose routine correspondence and reports. Ability to edit documents for grammar, punctuation, etc. Knowledge of departmental policies and procedures. Time management and organizational skills. Ability to withstand varying job pressures and effectively prioritize related tasks. Demonstrated interpersonal and effective communication skills that promote cooperation and teamwork. Ability to work with credibility and effectiveness with medical and administrative staff. Knowledge of payroll and personnel policies and procedures. Ability to work in a fast-paced environment. Ability to work with confidential information. May require demonstrated sixty words per minute keyboarding skill. Engage in culturally competent practice using demonstrated knowledge of the traditions, values, and customs of the Latinx community.
